Insurance and Forms

We proudly partner with Capital BCBS, Highmark BCBS, and Aetna health plans.  For those with other insurance plans which may provide out of network benefits, for a monthly fee our administrator can bill on your behalf.  

Its important for clients to know their health plan's patient financial responsibility - deductibles (and status thereof), co-pays, and co-insurance.  We will gladly provide patients an "explanation of benefits" which we can run with your insurance plan information in advance of the first appointment.  Fees apply.
